Eligibility and amount of coverage will depend on each eligible customer’s/patient’s insurance coverage. Based on an average insurance coverage of $248 per 2 years, you may be able to receive up to a year’s supply of select monthly spherical contact lenses (Opteyes, Air Optix and Acuvue Vita), subject to your vision plan and available balance. Subject to your insurance coverage, you may choose to use your vision insurance benefits towards the cost of prescription eyeglasses, prescription contact lenses and/or eye exams. If any prescription eyeglasses, contact lenses or eye exams are not covered or not fully covered by your vision.
When buying a single pair of glasses from our $149 or above ranges, you can choose between Standard progressive lenses at no extra cost, or $50 off one lens option. $50 discount is deducted before taxes, has no cash surrender or other value and cannot be applied to past transactions. Subject to suitability. Scratch-resistant treatment is included. MiYOSMART lenses, other lens types and extra lens options are available at an additional charge on all pairs purchased. Excludes safety eyewear.
